Carers Emergency Plan
What is this service?
Please ensure you have completed a Carers Emergency Plan with Essex County Council Adult Social Care Team - they will issue you a Carers Emergency Card that you must carry with you at all times and the card will have a single point contact telephone number and a personal identification number unique to you - should you suddenly by unable to fulfil your caring role anyone can call the number on the card and will alert the Emergency Duty Team at Adult Social Care so plans can be put into place for your loved one.
